POSITION DESCRIPTION The Clinical Manager , in collaboration with the Administrator/Clinical Supervisor, is responsible for the provision of high-quality patient care for home health clients in one-two branches.

· Provide intermittent skilled nursing visits, including SOCs and ROCs, as needed · Participate in on-site training for clinical staff · Communicate with internal marketing and clinical teams, physicians and facilities to collaborate interdisciplinary approach to promote optimal patient care · Assist in facilitating team case conferences and participate in management and staff meetings · Responsible for completion of in-home annual and as needed competency and evaluations · If designated, performs the role of an alternate administrator in their absence · Excellent auditory, verbal, and written communication skills · Provide clinical leadership and support to field staff to ensure high-quality, compliant care · Conduct 10% chart audits quarterly and as necessary for defensive documentation and compliance to standards · Review and complete OASIS workflow including correcting errors, validating reports · Complete in-home annual competencies, evaluations, and supervise case management communication · Promote team members' professional growth and serve on Performance Improvement and other committees · Assist administrator in collecting, analyzing, and reporting clinical data to improve overall clinical and branch performance · Ensure alignment with agency policies, procedures, and professional standards · Assist the Administrator with staff orientation and maintain communication with physicians, referral sources, and the broader care team · Carry out additional duties as assigned by the Administrator REQUIRED KNOWLEDGE, SKILLS AND EXPERIENCE · Current unrestricted RN license in the state of practice, or eligibility per Nurse Licensure Compact (NLC) guidelines; compact license preferred · Active CPR certification required · Must maintain a valid driver’s license and auto insurance · 2+ years’ experience in home health preferred · Experience completing home health patient visits including admissions with OASIS documentation · Proficient in case management and development of individualized care plans in compliance with regulations required · Experience in case management and developing individualized plan of care review preferred · Strong understanding of OASIS documentation, home care best practices, and Medicare compliance · Experience completing patient care within the home health environment i
